RN Medical Home Coordinator -Full Time

Position reports to the RN Clinical Supervisor with input from physicians, nurse practitioners, or other health care providers as a member of the multidisciplinary healthcare team. Responsible for contributing to the smooth delivery of patient care. Develops appropriate professional relationships that incorporate respect and integrity with patients, families, and colleagues.

Essential functions:

Medical Home

  • Work alongside primary care providers and staff to identify patient with complex needs

Care Coordination

At West Cambridge Pediatric and Adolescent Medicine behavioral health is an integral part of our primary care services.  Our primary care providers work hand-in-hand with our team of behavioral health clinicians to help the children and families we serve. 

The successful candidate will join our clinical provider team to deliver fully integrated behavioral health services to our patients across the pediatric and adolescent age ranges. Working within our family-centered medical home, the behavioral health clinician will:

Woburn Pediatric Associates is looking for a full-time Patient Access Coordinator – for our NORTH ANDOVER office.  As part of our Patient Access Group, you’ll actively contribute to the efficient operations of our office. Pediatric and Adolescent Medicine is a well established private general pediatric office in Western Massachusetts with 2 office sites:  one in Wilbraham and one in Belchertown.  Our current staff consists of 9 pediatricians, 1 nurse practitioner, and 1 PA.  

We serve patients in the suburban community outside of Springfield, Ma.   Our patients are usually admitted to Baystate Children's Hospital, which has a NICU, a PICU, Hospitalist coverage and outpatient pediatric specialty services.   We also utilize Connecticut Children's Outpatient Specialty clinics in South Hadley, Ma and the main campus in Hartford, Ct, which is under an hour away.  UMASS Memorial is also less than 1 hour away, and Boston area hospitals 1.5 hours away.  Our patient population is diverse, giving our pediatricians opportunities for assisting families from many different cultural, socioeconomic, and educational backgrounds.   An outpatient lab is located less than a mile away from each office site for easy access for our patients.

The office has a growing Medical Home  and 2 Behavioral Health Integration Providers to assist our families with complex medical, psychiatric, and emotional issues.  We have a Psychiatric Nurse Practitioner Consultant to assist with more complex psychiatric care.  We have many experienced licensed nurses dedicated to telephone triage and clinical support to providers.  We have an energetic and caring support staff to help serve our patients’ needs in every way.
